Practice & Cases

• Outpatient primary care focused on adults age 65 and older
• Manage diabetes, hypertension, hyperlipidemia, congestive heart failure, coronary artery disease, and mild cognitive impairment
• Nurse Practitioner, Medical Assistant or scribe, and Registered Nurse support
• No procedures required

Schedule

• Monday through Friday
• 8:00 AM to 5:00 PM clinic hours
• Weekday-only coverage
• Outpatient practice with no call

Requirements

• Must be an MD or DO
• Active Illinois license required
• Board Certified in Internal Medicine or Family Medicine
• DEA required
• Spanish fluency required

• Mileage reimbursement provided
• Lodging, airfare, and rental car are not provided
